Dear Enrollee:
This letter is to inform you of an update to your medical and pharmacy benefit plan based on Federal legislation concerning the Patient Protection and Affordable Care Act of 2010 (PPACA).This should act as notice of updated plan language under the new requirements. Please retain this notification with your summary of plan benefits.
Under PPACA, certain women’s preventive services must be covered by employers without cost-sharing. Aultra will implement all required coverage for women’s preventive services for our Self-Funded Non-Grandfathered Plan effective with our Plan renewal after August 1, 2012. The new women’s preventive services include the following:
Contraception and contraceptive counseling*– All Food and Drug Administration-approved contraceptive methods, sterilization procedures, and patient education and counseling for all women with reproductive capacity.
*You must have a written prescription from your physician. If a generic version is available, the generic version will be covered at no cost to you. If a generic version is available the name brand will be subject to cost-sharing. If there is no generic available, the brand name will be available with no cost sharing.
Breastfeeding support, supplies and counseling– Comprehensive lactation support and counseling, by a trained provider during pregnancy and/or in the postpartum period, and costs for renting breastfeeding equipment.
Screening and counseling for interpersonal and domestic violence – Annual screening and counseling for interpersonal and domestic violence.
Counseling and screening for human immune-deficiency virus – Annual counseling and screening for human immune-deficiency virus infection for all sexually active women.
Counseling for sexually transmitted infections– Annual counseling on sexually transmitted infections for all sexually active women.
Human papillomavirus (HPV) testing– Human papillomavirus DNA testing in women with normal cytology results. Screening should begin at 30 years of age and should occur no more frequently than every 3 years.
Screening for gestational diabetes– In pregnant women between 24 and 28 weeks of gestation and at the first prenatal visit for pregnant women identified to be at high risk for diabetes.
Well-woman visits – Annual well-woman visit for adult women to obtain the recommended preventive services that are age and developmentally appropriate, including preconception and prenatal care.
Immunizations – Additionally, Aultra and Independence of Portage County, Inc continue our mission of providing you with the best possible coverage by expanding your pharmacy benefits to now allow you to obtain your influenza, shingles*, and pneumonia vaccines at any participating retail pharmacy.
*Must be age 50 years or older and have a written prescription from your physician.
